Mechanism of Action of Sulfadoxine and Pyrimethamine Combination

This combination of Sulfadoxine and Pyrimethamine works synergistically to treat malaria by targeting the folate synthesis pathway essential for the parasite’s survival.

  • Sulfadoxine: As a long-lasting sulfonamide, sulfadoxine competitively inhibits dihydropteroate synthase, an enzyme critical for folate synthesis in the parasite. By hindering folate production, it prevents DNA and RNA synthesis, limiting parasite replication.
  • Pyrimethamine: This antiparasitic agent inhibits the enzyme dihydrofolate reductase, preventing the regeneration of tetrahydrofolic acid from dihydrofolate. Tetrahydrofolic acid is necessary for DNA and RNA synthesis in many species, including the malaria-causing protozoa.

Dosage (by Weight):

  • 5-10 kg: ½ tablet
  • 11-20 kg: 1 tablet
  • 21-30 kg: 1½ tablets
  • 31-45 kg: 2 tablets
  • Over 45 kg: 3 tablets

Due to potential side effects, this medication is no longer recommended for routine malaria prevention but is reserved for treating serious malaria infections or as a preventive option in areas where other treatments may not be effective.
